I understand and agree to the
following guidelines as a participant of the ASB Program:
- $50.00 deposit is not refundable under any circumstances.
- Withdrawal before December 14, 2005 will entitle you to 50% of your payments, excluding deposit, provided
your spot has been filled by someone on the waiting list.
- All payments must be paid on time according to the schedule below.
- You will be required to attend all trip meetings, participate in all fundraising events, and attend at least one volunteer
work project prior to spring break.
- The ASB ’06 leadership team has the right to terminate a participant as they deem necessary.
All payments will be made to your trip leader on or before the dates listed below.
Trip meeting dates will coincide with the payment dates.
By signing
this application, I understand that I will forfeit my right to participate in the trip if I miss more than two pre-trip meetings
and I do not make payments on the time indicated.
